The Clinton County
Healthy Living Partnership
(formerly The Women's Health Program)


One of the most important things you can do to live a long, healthy life is to get your yearly screenings. The Healthy Living Partnership is a grant funded program that provides cervical and breast screenings FREE to uninsured and underinsured women.

It is important to get your screenings because cervical cancer tends to occur in midlife. Half of women diagnosed with this cancer are between the ages of 35 and 55. Also, the chance of a woman having invasive breast cancer some time during your life is about 1 in 8. The chance of dying from breast cancer is about 1 in 33.

What Does the Program Provide?

The program provides to eligible participants:

• Yearly Mammogram (for women 40 or older)
• Yearly Clinical Breast Exam
(for women 18 or older)
• Yearly Pap Test
(for women 18 or older)
• Yearly Pelvic Exam
(for women 18 or older)
• Education about breast and cervical cancer
• Education of Breast Self Exams
• Health Care Referral, if necessary



How Much Does it Cost?


The Healthy Living Partnership is FREE to all eligible women. For women with high deductibles your insurance will still be billed and co-pays will be your responsibility.


What Doctors Can I See?

All local OBGYNs participate with the Healthy Living Partnership as well as Northern Adirondack Planned Parenthood, Women’s Imaging Center, Community Link Mobile, and some local physicians.

• New York State Health Department



Am I Eligible?

 A woman is eligible if she…

• Is not currently in treatment for breast or cervical cancer
• Has not had a mammogram in the last 12 months (for women 40 or older)
• Has not had a pap test in the last 12 months (for women 18 or older)
• Does not have insurance or
• Has insurance but it does not cover mammograms or pap tests or
• Has a high deductible or
• has Medicaid with a monthly spend down

 
 


Program Contacts


The Healthy Living Partnership is run by a program coordinator, a data/case manager and an outreach educator.

The data/ case manager will answer any questions on eligibility, make you an appointment with a local doctor and handle all paperwork involved. If you would like to contact the data/ case manager please call 562-7112.

The outreach educator can provide education materials as well as give presentations on the program or on breast and cervical cancer to your workplace or community agency. If you would like to contact the outreach educator please call 565-4993.
