How Much Does it Cost?
There is no charge for our initial home visit to
assess for home care services if the patient is not admitted to our
services.
There is a charge for our services. Payment is made from
various sources, such as Medicare, Medicaid, Veterans, Blue Cross/Blue
Shield, Worker’s Compensation, Liability, Major Medical, NYS Empire Plan,
etc.
We will bill your insurance for you. You will be asked to
supply us with signed insurance forms. If you have no insurance, you may
apply for an adjusted fee. However, you will be responsible to pay (at the
adjusted fee) for any services rendered which your private insurance refuses
to pay.
Medicare, and many of the private insurance, will cover
services in full for a limited period of time. The patient has to be unable
to leave their home and be medically unstable as documented by the doctor
and the public health nurse. Services are intermittent, meaning 2 to 3 times
a week depending upon your condition.

Availability of Services
Requests for service may come from doctors or
hospital discharge planners. All clients need a local doctor. Home visits on
weekends and holidays can be made by prior arrangement. Services are
available to any resident in the county. Detailed travel directions are
important. The duration of home care services depends on the patient’s
changing condition and needs.
Aide Services are available after the nurse has made the
initial visit and has requested service. Aide Service is based on nurse –
assessed need and availability of aides by the Contracting Aide Agencies.
The aide agencies initially provide as much time as
possible until the maximum time requested is available. You may experience
three types of aide changes:
1) Change in aide personnel
2) Change in time of arrival
3) Change in amounts of time received
All services are supplemental to the physician’s care
plan.
