How is Eligibility Determined?
In order to be considered for financial
assistance, a child must be:
Under the age of 21
A resident of Clinton County
Medically eligible
Financially eligible
Not Medicaid eligible
Have private insurance
What is Medical Eligibility?
A physician’s diagnosis report and medical data are reviewed by PHCP staff
to determine medical eligibility. Some of the conditions covered include:
Birth defects
Cancer
Cerebral palsy
Congenital abnormalities (such as cardiac disease or spina bifida)
Convulsive seizures
Hearing disorders
Orthopedic conditions

What is Financial Eligibility?
If your child meets the
medical requirements, and your family is seeking financial support, your
financial situation will be reviewed to determine the amount of assistance
that can be provided to you. All reviews are kept confidential. By law,
private insurance must
be used before PHCP funds can be issued.

What Services Might be Covered with PHCP?
Some of the services
PHCP might pay for include: Hearing aids and the co-pays for
physician’s office visits, medications and medication supplies. All services
covered by PHCP must have prior approval from the PHCP staff.
