CHILDREN WITH SPECIAL HEALTH CARE NEEDS PROGRAM

             


The Children with Special Health Care Needs Program (CSHCN)

 Children With Special Health Care Needs (CSHCN), are those children 0-21 years of age that have, or are suspected of having a serious or chronic physical, developmental or emotional condition. This program is designed to improve the health and care needs of children who meet the eligibility criteria.

The Physically Handicapped Children’s Program (PHCP), a segment of CSHCN, assists eligible families by reimbursing for health services provided to their children.

What Services Are Available?

Here are a few examples of some of the types of service available:
  • Help designing and building a ramp for a wheelchair bound child
  • Referral to summer camps for diabetic or asthmatic child
  • Assistance in finding after school care for a blind child with
    mobility training
  • Coordinating tube feedings or other care with school nurse
  • Provide information about support groups, respite, financial resources, equipment, etc.
     

How Much Does it Cost?

 There is no fee for participating in the CSHCN program. It is funded by a New York State Department of Health grant and is available in every county in the state.


 
Who is Eligible?

 Children from birth to 21 years of age and their families who reside in Clinton County are eligible to participate in this program. Referrals are accepted directly from families or providers. Children may only need services intermittently at key developmental points or during transitional times. All services are provided with parent consent and input.
