The first link is to the NYS DMV form MV-15 which may be mailed in, or to obtain it in a more timely manner, bring it directly to your local N.Y.S. Department of Motor Vehicle office and obtain your abstract in person.

A driving abstract must be obtained before submitting the Application to Reduce Traffic Infractions form. (See Instructions Sheet for full details)
